I have no fucking clue, but I've gotten that award twice.

Are you in high school and now going to college? Or have you been at one colllege and are now trying to move to a bigger university? If it's the former, you can get that sort of thing from your school's administrative and/or counseling office. If it's the latter and you need those things from a college, you go to the registrar, which should have a transcript office. In my experience, Dean's list isn't usually something that's added to your transcript. It's just a designation based on a semester's grades according to whatever academic standards your learning institution has specified for that honor. Once you finish a degree, if it's with honors, high honors, or highest honors, that goes on your transcript.
what happens if u got suspended once in gr. 10 and once in gr.11, could you still go to university?
