alright so me and my girl have been gettin it on right but i always hurt her(vagina) and i dunno what im doing. Sex shouldn't hurt all the time. I would insist on her seeing a doctor. She may have some problems that she may regret ignoring. I would also tell her to tell the doctor about how it hurts to have sex. My first thought was lubrication, but if you say you practice foreplay and she is wet everytime, then the next step should be to make an appointment with her doctor.

Have her ask her OBGYN (female plumbing doctor). My wife had similiar problems, and doc said that she had a tipped uteris, and it ultimatlely got fixed when she had kids.
