I said "revolutionary Spain and Ukraine", not present-day capitalist Spain and Ukraine, that is, the anarchist communist communities of the Spanish Civil War (before they were crushed by the Fascists) and the Russian Revolution in Ukraine (before they were crushed by the Bolsheviks).
Of course there is incentive to becoming a doctor. Some people's abilities and interests lie in the field of medicine, and they actually feel a sense of satisfaction in helping others and saving lives (like the brave members of your local volunteer fire department).
i have looked into it, probably not as much as you though...but i still don't buy it...it all consolidates wealth and power into the hands of a few...and socialism just leads to fascism.
once you consolidate the wealth and power and totally destroy independent business with endless regulations and taxes (which is probably not what you advocate, but it comes along with socialism from what i've studied, ie fabian socialism)...then you get the fascism.
it's all the same people using both systems to control people. we're now starting to see the institutions of control almost completely in place to bring dictatorship here...
but there is no left and right though...there is tyranny and freedom. i'm just rambling now...i'm smoking some good ganja. but i'll look into it some more.
It's rather unfortunate that people's impressions of socialism have been tarnished by the brutal Marxist régimes that resulted when people tried to combine anti-capitalism with centralized government (resulting, of course, in the appropriation of all property into the hands of the government bureaucracy). If there is any system that concentrates wealth and power into the hands of the few, it is capitalism. The wage gap between the rich and the poor has been growing steadily, and has doubled since the 1970s. One percent of the American population owns 38% of the nation's wealth. If that isn't concentration of wealth into the hands of the few, I don't know what is.

that centralization of wealth isn't due to capitalism, at least in my opinion, it's not free market...but monopoly capitalism, a bit of irony or sarcasm in the name. it's all consolidating now, slowly and slowly...soon, we'll have one place to shop, once place to do this, and that, and it'll all be federall controlled with national id cards and such...elite families who have schemed and killed and lied to attain their power status...and they're all over the planet...more akin to feudalism and mercantilism...
i'm looking at it from the perspective of how this country was founded, and the ideas espoused during the founding of this country, in the federalist and anti-federalist papers...we're seeing the same tyranny we did before the revolution...basically exactly the same, as far as corrupt sheriffs and regional governments that create taxation without representation and no property rights...etc. the corruption is everywhere...
i wonder if any of the figureheads in the spain or ukraine had any kind of part in what you're describing, but that's something i'd have to spend a lot of time looking into...at this point in history, it's almost entirely those in power now that lied and schemed and offered false opposition or maybe even attacked themselves to blame on enemies in the past to their own populations to achieve their power, and who have sold out their country.
but now i forgot what we were talking about...so i'll stop now...this is some good weed.
