I agree.

I'm also disappointed that they couldn't come up with something besides having the patient provide their SSN. As a caregiver, I wish patient's didn't have to give that info out.

I've been put through the hell caused by identity theft and the more I can keep my personal info to myself, the better off I am....hopefully!

I won't wish it on my best enemy.

HEADS UP FOR NEW PATIENTS: Old (yesterday) forms will only be accepted until July 9th. After that, they will be returned requiring the new app and cert be submitted. Meaning you'll have to make at least 2 Dr. appts to resubmit. Bogus way to retro this as they won't even open these envelopes for at least 3 months.

I emailed Mr Salley at the CDPHE about this because I thought it was a crock also...I got a response from Bob O'doherty that as long as the doctor sig. is pre-July 1st that patients still have 60 days to send in the "old" forms.

There was a similar scenario last Aug. when new forms were released requiring a notary signature and stamp. Many of the "old" forms were returned to patients in Sept. and Oct. for lack of a notary, despite the fact those forms had no notary designation.
I don't know, there's obviously conflicting info. here based on the email someone else referenced.

That's exactly what I was looking at when I clicked on Mr Salley's email to contact them. If you'd like to contact them yourselves I'll post their email addresses. I inquired because I got my renewal on 06/30 and hadn't sent in my doc's as of 07/10. Some doctors are willing to swap out the older forms for newer 1's but according to mr O'Ooherty it's not necessary as long as your doctor sig was from June 30th or earlier (if it was then you have 60 days from your rec to send them in as stated in Amendment 20).

Ok... I sent my own email to Marc Salley and also got a response from BOB O'DOHERTY <[email protected]> Here is what he told me.


You can use your existing physician certification form as long as we receive your application within 60 days of when the doctor signed.

You must use the new patient application form.

I have old forms dated June 11 but I havn't been able to afford to send them until this week. Does this mean I will have to pay for another doctors visit to get the new forms because its July 14? Or do the forms just have to be dated before July 9? I'm on a tight budget as it is.

That IS kinda weird. I emailed again to ask about both forms, that kinda seems like you need a new registry app which IMO would still be B.S.....I'll let you guys know when I get a response.

I agree... I also sent another email trying to clear this up.

So I will still have to pay to have it re notarized? Or do I just fill out the new form and send it with the old one? If I have to get it re notarized is the conflict in dates going to be an issue? Will you be applying credits to reinstatement fees for the extra costs people are going to be acquiring? Alot of us are on a tight budget.

Havn't heard back yet.

By the time a patients paper work is returned requesting a new application form be submitted, the Phys. Cert will have expired. This is positively byzantine and is not acceptable. Patients, care givers, 'center" owners seem to be held to one legal standard, while the CDPHE is held to another (not complying w/ time frames mandated by Amendment 20) Just my opinion.

Here is the response I got to the last email...

You have to have the new form notarized.

The date that you sign the form must be the same as the date the notary signs the form.

The state offers no credits.

I think he misunderstood my question about the dates...

You will need a new registry form and it will need to be notarized. :mad: I'm waiting to hear back about the conflicting dates between the doctors form and the new registry form.
