I've been put through the hell caused by identity theft and the more I can keep my personal info to myself, the better off I am....hopefully!

I emailed Mr Salley at the CDPHE about this because I thought it was a crock also...I got a response from Bob O'doherty that as long as the doctor sig. is pre-July 1st that patients still have 60 days to send in the "old" forms.

There was a similar scenario last Aug. when new forms were released requiring a notary signature and stamp. Many of the "old" forms were returned to patients in Sept. and Oct. for lack of a notary, despite the fact those forms had no notary designation.
I don't know, there's obviously conflicting info. here based on the email someone else referenced.

That's exactly what I was looking at when I clicked on Mr Salley's email to contact them. If you'd like to contact them yourselves I'll post their email addresses. I inquired because I got my renewal on 06/30 and hadn't sent in my doc's as of 07/10. Some doctors are willing to swap out the older forms for newer 1's but according to mr O'Ooherty it's not necessary as long as your doctor sig was from June 30th or earlier (if it was then you have 60 days from your rec to send them in as stated in Amendment 20).
