Getting Started Without Medical Records
I have my card in Colorado at the moment but will be moving back to NM shortly and was wondering what is the best way to get started. I don't really have any prior medical records as I have never really had the insurance or the cash to visit the doc too many times. Any help would be super appreciated.

Hey Mike!
Well, it depends upon your condition. The best thing for you to do is check the NM Dept. of Health website for the qualifying conditions. Once you know you have one of the 16 listed, you have to find a NM doctor. Depending upon where you live, that may or may not be an easy thing to do. If you are in or near southern NM (Las Cruces) there is a group called Full Circle Health that will be able to help you. If you are in the Albuquerque area there is another group and I believe they advertise, so you shouldn't have any problems finding them.

Looking at the current qualifying conditions it looks as though I would have to via PTSD or Chronic Pain. If I can get a p-logist to diagnose me with PTSD does she or he have to fill out the recommendation papers of can I take the diagnoses that they gave me to a "cool" doctor and have them sign off?
In order to try to get my card I can go anywhere in the state.

To my knowledge, you have to have your paperwork signed by a psychiatrist or psychiatric nurse practitioner for PTSD. But only the one signature. For chronic pain, you need a primary care AND a pain specialist.
