They are ALL OVER LA here.. You have to go to a doctor and get a medical license. Then they give you a list of places, and you can go buy.

Depends, there are doctors out there that will take your word for anything. Its generally around $150. You basically go in, pay the fee, and tell them "marijuana helps me sleep", and you get your card. Some doctors actually only give it cancer patients, and anerexia victims.
Dont ask me for any numbers BTW...

Don't make up symptoms to try to get medical acceptance. Be honest with your doctor. Tell them exactly how it helps you and why. Remember these places are for medicinal users...people that use it for specific things.
Depending on your doctor and the state/province you're in it may go either way.
Here in Ontario, this is how the categories are broken up.
Category #1 is for people who are suffering from AIDS/HIV, arthritis, cancer, crohn's, epilepsy, fibromyalgia, glaucoma, hepatitis C (active), intractable pain, multiple sclerosis, muscular dystrophy, paraplegia or quadriplegia. For Category #1 Applications, your doctor (licensed MD or ND) must provide a diagnosis of your condition (among those listed above). He/she need not even know what the diagnosis is for - and thus does not even have to be supportive of your self-medication with Cannabis (although a supportive doctor is, of course, preferred). We feel that for these conditions diagnosis is more than sufficient as far as doctor support goes, and we have found it necessary to allow for membership without explicit doctor support due to ignorance and confusion among many medical professionals.
Category #2 is for people who suffer from an ailment not covered by Category #1, and for whom a doctor (licensed MD or ND) feels that marijuana could be beneficial. Due to the huge range of illnesses for which Cannabis has been shown to be beneficial, we allow for membership no matter what the illness as long as a doctor is willing to state that he/she feels that 'cannabis may be beneficial' for you.
Category #3 is for people who have received their government (Health Canada) issued exemption. This allows for expedited membership for people who have already jumped through the hoops required for Health Canada MMAR (or Sec. 56 or other) exemption.
Note, category 2.
Hope this helps.
Bob.
