I'm a Buddhist, I've been meditating for over 20 years and things like ego death are of particular interest to me.

It happens after certain kinds of transcendental experiences, particularly to meditators who are on the path to enlightenment. I have to speak very broadly here or this would be long and dull, but enlightenment is not just a state that someone might reach, but an understanding about the true nature of reality. However, it isn't the understanding that 'gives you' ego death. You don't suddenly understand reality and think, "ahh, no need for ego then." It goes after enlightenment, it just dies of its own accord. You lose desires, you dont feel the need to fulfill anything or go anywhere. But not in an empty, lazy way, it's a positive state. If people insult you it means nothing, how you look is nothing...etc..it's a whole set of things that happen together. I would be interested to see if it has happened to you, it's certainly not impossible.
