off the back of your scalp... or arm hair, any where, there is hair, is a folicle.. so.. usualy from the scalp.

Anywhere. I've heard of people shaving their heads in hopes of beating the drug tests but they'll just take hair from your arm, you leg, your pubes.
unless you can effectively wax your whole body, your only option is masking shampoos and things like that.
personally i'd rather fail a drug test than wax my whole body.
