When you're inhaling the smoke through your mouth, leave your mouth open a little bit and don't inhale very strongly at all and don't take the whole puff of smoke into your lungs. Basically, it's just a matter of letting a little "tongue," or mouthful, of smoke remain uninhaled so it wisps out of your mouth visibly before you finish the final part of the inhalation and and suck it back in. This was a much-desired talent to have when I was in college, and so we practiced it in front of the mirror, which is helpful. Smokin D's right. Practice is necessary. And even with lots of that, it's not something that happens every time you try it.
