As to the whole expanding in your lungs thing, that seems kinda sketch. More likely, it doesn't hurt as much because YOU'RE NOT INHALING THREE HUNDRED DEGREE BURNING ASH AND TAR like you are with a pipe; thus, ice hits tend to be a lot less irritating.
