Can some one explain what 'paper chain of custody form' is, as my test does NOT require it.
It is required by the DHHS for all drug tests to be accompanied by a CCF form. The form will outline your personal info, the type of test to be performed and spaces for the MRO to fill out upon completion of the test. Without the CCF, sample mixups and/or misplacements would run rampant because the lab would not be able to link the sample to its donor.
