Longtime grower. Can't figure out this one. Pic included

I had this problem about two months ago and lost 3 plants that were all over 30" tall. I thought the return pump in my controller malfunctioned and kept the garden flooded and a few had suffocated so I switched it out. The whole ebb and grow ran 56 plants and of those 56 i lost 3 of the larger ones. Now it has happened again. I can't tell if anything has malfunctioned or what happened but its the same scenario, this time it's 5 of my plants.

I run a cap ebb and grow 2.5 gal and a 5 gal monster (same res) with botanicare pure blend pro grow as we'll as B-52.
