Could mean whether you're helping them around the house with laundry, dishes or such or that you are driving them to doctor's appointments or to get groceries. I think some of these states are going to try and push for actually trained caregivers that do more than just supply cannabis to them. Its why legalizing with government regulations is always the worst way to go. They will regulate, tax and fee the shit out of you as they figure out ways to do it.
