If you feed to often and don't flush regularly, you end up with what appears to be the situation you are in now, nute lockout. Yep that is the cause.

Also, if you are going to use water that has been filtered, you are going to need some Cal-Mag. Not filtered, only RO or distilled. Filtering does not remove micronutes
