That's what I do. I have two identical pairs of black, Fruit of the Loom briefs, the kind that come in a plastic tube like 4-5 to a tube. They never have two black in the same tube you might have to buy two sets but hey, now you got a bunch of new drawers.

These underwear have double material that spans from the band in the front all the way down past your jewels on the inside. The double material is roughly the size and shape of a jock strap/cup deal. So there are two seems that span from the band down on both sides. It is like a closed up pocket.

All you need do is get a seam ripper or similar object and carefully on one or the other side (best for it to be your dominant hand side) remove the seam from top down. Go only as far as you have to so that you can easily slip the bottle with attached, banded heater in and out easily. Use black thread and needle to assure your seam holds tight because the thread you ripped earlier will continue to loosen. It's really easy.

You now have a crotch pocket in one pair on the inside where it cannot be seen from the outside. I then put the other pair over top. This way in case the physical calls for a hernia check which most of mine have when you drop your jeans and two pairs of underwear down to your knees, it is 100% impossible to see it tucked away in your new pocket. Pull your pants and underwear all up in one motion and go to the john to pour it into the cup. Just jock it back in your underwear and be on your way. Easy as pie.
