Need help with my first grow, plant is stunted / yellowing

What happened to questions 15-18 on the form? We need to know what you have been giving it.

15) Any additives or tea's? nope
16) Are your ph levels stable, or do they fluctuate? this month I've been watering with PH corrected water, before that I really don't know.
17) What is your ingoing water's ph? ...your runoff ph? I'll check out how to do this and post back.
18) Do you foliar feed? nope
