6. smoke away, adjusting the carb size as needed

Uhhhhh, to be honest, I'd stay away from that method. The hot temperatures from the joint might vaporize the plastic material in the straw, causing worse problems with the lung problems. I'd recommend the thread starter to go out to his head shop, and buy a glass downstem and bowl. With that glass downstem and bowl, you can practically make any plastic bottle or even glass bottle (If you have a glass cutting drill bit, which I had) into a water pipe/bong. Trust me and invest in the $20-$30 glass pieces, and enjoy chemical-free smoke.

How to make the bong with the glass downstem/bowl, just cut a hole into the bottle, and put the glass downstem in, if you do it right, you won't need any sealing. But if you made the hole too large, use gum to seal the hole, trust me, it's pretty ghetto, but it works, and it's readily available.
