Update: how to approach a doc

Well I took everyone's advice and was honest on approaching my doc......and..........
She signed of my paper work! Woo hoo! Since my condition is autoimmune mediated arthritis...and she was a rheumaologist, I don't need to send medical records, right? Just my consent to release medical info, right? Also should I put in for my production license? THANKS for y'alls advice.....I'm pleased as peas!

    Way to go, lovemyhens!

You are right, you don't need to send records. Just the paper that the doctor filled out, and the release, and the personal production application. Yes, you should go for that, too. You just can't tell people...
