Gathering information

I am applying for a medical marijuana card for my back. I was in a car wreck a couple years ago which caused my hip to rotate the wrong way and it can't be fixed (due to me being double jointed). Resulting in the most severe back pain. Anyways I have medical records from when I was at the ER, Chiropractor, Physical Therapy, and my doctor (nurse prac.). It's been about a year since I have been to the doc cause I am on unemployment, but the chiro, PT, and NP knows that my hip can't be fixed.

Will those medical records work?
Do I need to get sum more recent medical records?
Also I have a N.P. where can I get a Doctor to sign my papers?

That sounds like u should qualify for "Cronic Pain." I got my head bashed with a baseball bat almost three years ago and lost a finger... Recently moved out here, went to the doctor and took him some hospital records, told him I always have killer headaches, and hate narcotic pills cause they make me irritated... Then I mentioned medicinal cannabis and he agreed it might mitigate my symptoms... So long story short, he filled out my OMMP Dr. application, and I'm currently in my third month of waiting for my card... There you go, its not that big of a deal... Have u been to the OMMP website?
