What condition do you have?

I have Severe Chronic pain, my Specialist from Albuquerque sign my form but havent had any luck with primery care provider. He said the hospital board would not let him sign the form.

THanks for the help I was appoved by another Doctor didn't have to use the one you recommeded. Now if I can just figure out who the lea county supplyer is I will be doing good. THe DOH is of no help they gave me a list but no one from lea county is listed.
