I send a certified letter to both the patient and CDPHE and explain to the patient that they are responsible for changing their caregiver status within 10 days of receiving the letter. I also provide a link to the new caregiver form so they have no excuses.

I check the tracking online and make a copy of the confirmation that both parties have received my letters and staple that to the patient's paperwork.

I feel this covers myself. I can't make a patient change their caregiver status and that's why I send a letter to CDPHE also.
COzigzag Reviewed by COzigzag on . Caregivers dropping patients So how exactly does a caregiver drop a patient in order to be compliant to the 5 patient rule? Are there forms to fill out? Do they just pull 6 plants and call it done? How does the Dept of Health (or the cops) know when or if a caregiver is no longer a caregiver for a patient? Rating: 5
