I think the question is misleading and your answer is both correct and incorrect at the same time... depending on the chiropractor.

"Health care provider" is a loose term the DOH likes to use. Don't be fooled by the use of this word.
If you submit a recommendation from a straight Chiropractor, it will be declined. Chiropractors can NOT write prescriptions for narcotic medications.

If you submit the same recommendation from a Chiropractor who is ALSO a doctor or health care professional that CAN prescribe narcotics, your application could be granted (depending on all other factors).

Again not picking bones, just trying to help.
I know because I submitted a recommendation from my Chiropractor two years ago, and it was denied.
