Does anyone know what this one is like? It looks very legit - was in the original plastic wrapping and the guy that owns the shop has about 100 of them sitting there under the counter.

It has the bottle, lil hotties hand warmers, the vial with the beads, instructions and a rubber band. If I go in there with the correct temp will i be fine?

Sounds like you got a legit batch to me, just like any of the ones Ive ever ordered. BTW, the Canadian version was designed to beat labs not following protocol in Oregon. The vial with the beads is the Uric Acid, its the extra substance provided to cover the validity check.
