Doctors for PTSD
As I mentioned earlier, I was certified in May of 2010. Make sure that you go to the site below to get the most UP TO DATE forms before you try to apply.
Go to Medical Cannabis Program - Infectious Disease Bureau
Print out the "Participant Application Set" form and take it to your PTSD medical provider.
You can fill out the top of the form but there is a section for your medical provider to fill out.
Make sure the Post-Traumatic Stress Disorder condition is checked.
This is what my provider wrote in the section under "Written certification MUST be provided below" section:
"patient is under my care for PTSD 309.81. Benefits of marijuana outweigh risks"
MAKE A COPY of your completed form before you send it in!!!
You must also provide a clear copy of a New Mexico photo id or driver's license

Can I please ask you what CNP stands for?
I have been trying to find a psychiatrist who specializes in PTSD to make an appointment for my son.
I'm not sure how to tell which initials mean what following a doctors name.
