Other than that, there are a lot of Doctors that will write you a recommendation if you have a qualifying condition. Most recommendations are given because of severe pain. You should be able to find a number in the Westword. We have a Doc we recommend as well.

Once you have the recommendation, you send that, $90 and a copy of your ID to the state via certified mail. They have 35 days to reject you or issue a card. You can use a copy of your Dr's recommendation until then to make purchases. Make sure to save proof of mailing!
