I'd notify your caregiver and submit a change form to the CDPHE, your card will most likely have that caregivers name on it till you renew, but you will be listed on the reg as yourself and no caregiver if you submit a change form.

Yea, the caregiver is not the dispensary or its owner. I'm assuming he's just a regular grower that teamed up with the disp.

I found this on the CPHDE website:

"When there has been a change in the name, address, physician, or primary caregiver of a patient who has qualified for a registry identification card, that patient must notify the registry of any such change within ten days."

and

"It is up to the patient to notify a person that the patient has removed them as a care-giver. The Registry does not send any notice to the care-giver."

So it seems clear cut. (Smiles, I couldn't find the 30 day advance rule)

I just send a letter to my caregiver's listed address and then within 10 days fill out the 'Change of Address or Caregiver Form' and check 'none' for caregiver and have it notarized :cursing: and sent it in to the CPHDE?

I also seem to remember that I signed a paper at the disp stating that I would notify them also if I changed caregivers. Since they are closing does this really matter?

This is gonna sound like a stupid question, but will the air inside my airtight containers degrade the mmj? I was not planning on using any kind of vacuum jars.
