Go back and ask them to black it out. I know I went back to a couple shady places and got my paperwork back completely. At first they wanted to give me grief. I simply stated I know I'm not returning as a customer and so they will not longer be a caregiver. I threatened to make a few phone calls to my lawyer, law enforcement and Dept of Health. They found it quickly and told me to get the hell out. Of well burned a bridge but at least I know my paperwork isn't floating around anyplace where I don't feel comfortable. Mind you, it had been at least 4-6 weeks since I had visited this place so pretty sure they have no need for my paperwork anymore.
