I got my pain management doctor to fill out my application!!! I have an appt next week with PCM for the second signature and then I plan on turning it all in!!! I was a bit nervous asking my doc about this. He was the one to recommend this to me back in January but I did not want to jeopardize our relationship. The meeting couldn't have been farther from my imagination. I didn't even ask about it until he said Rating: 5
