I have been issude my card for chronic pain. My doctor and my PCM signed letters attesting I was a good candidate for medical cannabis and signed the DOH paperwork. No copies of MRIs, x-rays or the likes (though I have plenty).

Just the completed forms and letters from the two doctors is all that was required (for me).

I hope this helps and GOOD LUCK!
therealwrwhite Reviewed by therealwrwhite on . Less than a THOUSAND patients statewide???...please take a peek Hi all, I did not get many posts from my INTRO and thought this site was a bit dead. With that being said, I was shocked when my pain specialist brought up this subject. He said I was definitely a candidate for MMJ but will require a PCM signature.....I am pretty sure that it would be possible but looking at the amount of patients approved here in NM....I have my doubts. The fact that a med-prof brought this up after over a year of treatment out of the blue really got me thinking. Rating: 5
