My GF is looking to get her card...

Does the $150 fee include the $90 state fee.... or is the customer obligated for that in addition to the $150 (for a total of $240)?

Also, does she have to sign you over as caregiver? If she doesn't is there any additional costs/fees?

Let me know. If things sound good I'll have her call you tomorrow to set up an appt!

The doctor visit is 150 and the state registry is another 90. So you are looking at 240 full pay for docotor and registry.

This is what the doctor charges, I do not charge the doctor, and I do not see any of the evaluation fees. We work with the doctor, but we do not exchange any money. He is a very respected doctor who has been in the field for 20+ years and is one of the top emergency room surgeons in denver.

I do not offer any discounts on evaluation for signing CCF as a caregiver, however we will do 10% for all patients who do sign us over, and given that we keep some of the lowest prices in denver, your savings start to really add up fast.

There is no additional fee for her NOT signing us as a caregiver, but we wil still offer up the free gram / free edible with any purchase for new patients!!
