1. Don't know about dispensaries as I grow my own.
2. A care giver can not sell anything. It is all freebie or it is considered breaking the law.
3. Nobody knows about your card unless you tell them so the answer is no, it does not affect your insurance in any way. And the state can not tell anyone you have it either. They can tell the police ONLY if a valid request is made to verify you are a MMJ patient or grower. And that is only if you get in trouble while dispensing/using/transporting.
