You must be in chronic pain (lasting more than 3 mos.) 3. You must have medical evidence. (mri's, ct's, x-rays, surgeries, etc.) 4. You must be willing to pursue continuing treatment, mmj can't be the only thing you seek. You must be willing to try the meds, injections, physical therapy..... 5. You must be professional. Sorry, but face tattoos and mohawks, or rapper jeans below the ass and a big gold chain will be turned down for the card. At least look Rating: 5
