Questions about RN/LMHC medical records
I have a friend who has a qualifying condition, but the clinic would not accept the NP's documentation. Her doctor refused to sign off the nurses docs, so she had to find a new doctor and her file got flagged by the HMO she belongs to.
It has to be a licensed MD or DO.

However, all of my medical records and documentation about my conditions have been done either by an RN (for scripts) and a licensed mental health counselor.
