All you need is the recommendation/authorization form from him and any other paperwork they give you. Be sure to keep a copy on you at all time, one in your home, and where you may grow if you so decide, and one in your vehicle when you are transporting medication. That is what I have from them and that is what they will tell you also. No card unless they have changed to that.
gypski Reviewed by gypski on . Getting MMJ card, questions This Friday I have an appointment with Dr. Orvald in Bellevue. I have been told that I qualify for MMJ (chronic lower back pain). My question is; does he give me the MMJ card or do i take his recommendation to a counsel? I just cant afford to pay 200 to be seen, then denied for some reason. I read another post about Dr. Orvald no longer having his certification? Anyone know about this? Has anyone had a problem with this doctor? Thanks. Rating: 5
