new troubleshooting form input

what's good everybody? seeing as how a lot of people think the old troubleshooting form is confusing (i think newbs mouths are getting bigger and brains are getting smaller personally) i'm gonna go through and make a new troubleshooting form.

goldengrahm and i had one sometime back, but it's since been lost. and golden no where to be found. maybe he took off with our new form? (j/k, hope all is well gg!)

anyhow, this is your, yes YOUR chance to get some input for the new troubleshooting form. so if the new one sucks i don't want to hear about it! you had your chance to speak up!
