Suboxone from your doctor (I'm shocked he hasn't suggested this yet after SO much time!) and pot from your garden.

My best friend, who freed himself from a decade-long opiate habit, and HATED methadone, is now successfully using a low dose of suboxone with cannabis for the lasting side effects of the opiates. Knowing his experiences getting clean, it seems like you should at least look into suboxone treatment. Cannabis is also a time-honored way to get off opiates. There are countless anecdotal accounts of GIs returning from Viet Nam with heroin problems, or PTSD that led to them developing heroin problems, and then later getting clean by smoking themselves into oblivion to get past being dope-sick.

Best of luck to you and I will see you in the growing section!
