A hospital may do its testing in house instead of sending it to a lab but the process and regulations are the same if you went to a collection center. Immunoassay follwed by GC/MS for confirmation along with validity testing.

As far as the ramifications go, some companies offer rehab for first time offenses as part of a health care plan. Rehab is for quiiters though.
