smoking condition??

I was watching south park and then watched the martial arts film 'Ong Bak', which I remccomend to all. but anyways know that guy in south park who has a missing arm?
What is that thing he has to hold to his neck so that he can speak? Its kind of like a mic but its has a buzz sound to it.. and in the movie Ong bak that gambler also has the buzzy mic but also has a hole in his neck where he smokes his cigarette..

Can anybody tell me more about this 'condition'? I'm interested.

Dont know the wat the condition is called .. But dam that movie Ong Bak Is a brilliant movie.. In My top 5 favorite movies..
