I would say you don't need to disclose anything with the creatine, if they ask about your levels just say you are trying a new workout regimine or something.

Yes disclose the use of Vicodin if you have a valid perscription.

High creatinine levels are a non-issue to the labs because there are no upper limits established for creatinine. When labs check the creatinine, they look for low readings. As long as the creatinine level is over 20 mg/dl, its satisfactory.

However, this doesnt mean you should go crazy with creatine-loading. Too much creatine can result in long unwanted sessions, sitting on the potty.

They don't ask about prescriptions anymore prior to the UA. A MRO will call if they detect it and ask what pharmacist or Dr. you're using. They'll check to make sure you have a legal prescription.

Burnt and Killer are right on point. There is no upper level limit to creatinine clearance and you don't tell people at the collection site what prescription meds you are on. If you fail for a control prescription med, it's the MRO's job to verify it and keep it as confidential as possible. I forgot when the MRO can and can't disclose this info but basically they decided about 10 years ago it was pointless to tell the collector since the MRO would be the one verifying it.
