could i ask you, you took your medical records, and any other documents? and could i ask where abouts in pacoima? thanks.

No records or anything you just fill out a couple of forms on your health, family history stuff, and what is wrong, and if you currently smoke to treat the condition.

The doc looked me over in a min, asked me a couple of simple questions, and I was certified!

It's 125, but I got it for 115 with a coupon. Renewals are 80 I think.

what no med. records? i'm currently waiting for my copy to be made, if i may ask i thought records were required, but guess i might be wrong..

you dont need them but it makes your case stronger

i found a place near our home but he requires to fill about 7 pages of medical history, myself, family, etc... is that normal for all docs?

Wow that's a lot of paperwork, like I said I filled out out 2-3 forms. It really seemed like the doc at this place was giving them out like candy. He barely even looked at me.

You don't have to, but you can. I waited 2.5 hours, as I was a walk-in.

wow, 2.5 hours i wonder if my back and legs can hold out that long, i'm going to call them tomorrow and see if the appointment will be better, and let them know of my back and leg problems, and the other stuff and maybe he won't let me suffer to long in his office.. thanks.
