Since you have a legit medical problem, can get your records, and reside in California, you ought to have no problem getting a recommendation. My doctor requires picture ID, but the DMV has a non-drivers picture ID available. Get one and you are good to go, even with a fussy doctor like mine!
And it is not a "License", it is called a "recommendation". The card that some counties sell is not required to use medically, but many dispensaries will not sell to you without a card. The card is supposed to keep the cops off your tail, but there have been instances where card-carrying med users have still been hassled. Some dispensaries issue their own cards, from what I hear. I don't know what their ID requirements are.
