An M9A1 9mm Semi-Automatic pistol with silencer
<EDITED--synthetic adrenaline replacement> and <EDITED--powerful analgesic> syringes, tactical first-aid supplies, water purification tablets, and various small gear.

Of course the most important pieces would be a Japanese katana folded hundreds of times, a sharpening stone and oil.

If you think about it: there are places to get ammunition absolutely EVERYWHERE. And if its a post-apocalyptic scenario, then it simply becomes a matter of locating and obtaining said ammo.
