I know MY problem is, I used to hold hits and force so much blood to my head and added pressure that now my ears pop a lil when I smoke; doesn't hurt though, yet, so I'm good.
Do you hold your breath by closing your mouth, or do you sort of close your throat? I close my throat, and my mouth is sort of open a bit, so there is no way holding my breath can pressurize my head and pop my ears. I never really thought if there is a reason why I do it that way, I just do.

Also, sometimes smoke itself can cause ear infections, which is a totally different kind of earache than what I think we are talking about here. I knew someone whe smoed a tobaco pipe once every couple of days, not a lot of smoking really. Ha always was getting htese ear infections. He gave up the pipe, and now he doesn't get them. That probably has nothing to do with this problem, but it's just interesting.
