As far as psychotically unstable people murdering in the name of God... it is usually nothing more than a schizophrenic delusion.. Some part of their personality--or a separation of their personality--wants to do harm to another for whatever reason. They then have an internal monologue with the host personality, and the host claims to hear the voice of God telling them to murder whoever it might be.

As far as people who claim to hear God telling them with a spiritual reasoning, I think it is more or less an intuition. While conscious, they have slipped into a partial meditative state where their brain is performing on an alpha-wave frequency. I am guessing their subconscious mind has accessed the Akashic Records, or something of the sort, and very real information that they had no previous knowledge of, was granted to them. Edgar Cayce, an american clairvoyant, performed over 20,000 psychic readings for people, over 14,000 of which have been documented in detail. He performed medical diagnoses, foretellings of the future, tellings of fortune.. all while in a self-induced trance-like state. When asked one time under this hypnotized state where he got his information from, he replied "the Akashic Records." Of course, Edgar Cayce, being an uneducated man, had no previous knowledge of such things, and absolutely no medical knowledge, yet he diagnosed others and himself in great detail. Some diagnoses were unfamiliar even to the medical world at that time.
