this is for a future employer. and what is a "chain of custody form in that paperwork" ( i got papers telling me where to go, and what im being tested on)

I finally got the papers that tell me about my drug tests. and i am up for a non dot test. what is the difference between dot testing, and non dot testing? and also what is the cutoff level for Pinnacle Urgent Care?
