event horizon was creepy as hell...
but to me, one of the scariest movies i've seen (and i know it's not a horror flick or anything), was requiem for a dream. holy fucking shit i'm not doing hard drugs. ever. that was so scary, and the end is just heart breaking. i know that's not what you're looking for lol but in my opinion, that's one of the scarier movies, because of it's gritty reality...that's actually happening to people.
the descent was creepy as hell, i mean, watch it and imagine that happening to you holy fuck it made me jump quite a bit when i was watching it.

If you want gore and sickness id go for Hostel 1 + 2, the hills haves eyes (the remake).
I dont like how scary movies nowadays are just about who can make the viewer want to puke. To me this is not what a scary movie is about. I like really creepy movies. Like The Shining or Event Horizon. I remember this really creapy movie I watched once called Dead End. That shit freaked me out.

Used to be the Exorcist..bloody and scary, try Hellraiser, Wolf Creek, Haute Tension, Ju-on the Grudge, Audition or Ichi the Killer not really scary but insane and ultra-gory. Can't go wrong with classics such as Suspiria, the Texas Chainsaw Massacre or the Omen. Jacob's Ladder still freaks me out.
