Your eyes naturally shift from left eye to right eye when you looking at someone. Thats why when you're acting on film your supposed to focus on the closest eye to the camera.
I'd ask him, in a very sincere way, "What's the deal with your eyes ?". You might get an honest answer, and then you can explain that you're not quite sure how to look at him. Maybe he can help you.

Thats fucking funny i worked with a guy the same.He had eyes like a punter 1 each way so i just looked at his forehead because there were a few times when he was talking to me i ignored him because i thought he was looking over my shoulder.In the end i just said look mate sorry if i dont answer sometimes but your eyes are pretty uneven and i think your looking at something else.He was cool with that was all good
