I have recent DX for severe back pain due either nerve damage or thorasic spine injury. I know it requires a specialist to 2nd the Dx for chronic back pain as well as x-rays and MRI, which seems like a big hassle and quite expensive. I also have Dx for insomnia, Peripheral Neuropathy & Siatica and anxiety which I know are qualifying Dx as well but will I need to have more than one doctor or a specialist sign off on these or can I just take these records to an openminded practitioner? I recently had x-rays and labs done. I am currently scheduled for an MRI and possibly a bone scan. I also have over a year of RX documentation supporting my multipe DX. I recently did one of the online websites prequalification assessment which gave me a score of 19 and said all I needed to qualify was a 3. So what might my next step be, do I have enough? I'm so tired of taking medicines which don't help and make me feel terrible.
I'm sorry for asking so many questions but the information at the DOH website on the qualifiying conditions and records needed is somewhat vauge. I hope I'm not out of line for the MODS by asking for help on this matter. P.S. you have all that you need for a doctor to be able to sign off for you . Get your !@#$ moving and get approved
