Is crushed ice better? I would imagine the small and uneven surface mass would cool a liquid even faster.

I would use crushed.

i love to have crushed ice in my bong it cools the smoke the best

I think the best thing to do is to fill the bong with ice water, then put it in the freezer. That way, you can get the water almost completely frozen, then pull some massive fucking hits!
